Ascension St Vincent Clay Hospital provides free backpacks and school supplies to children and families in need, who may not otherwise be able to afford them.
This form can be turned in at Ascension St Vincent Clay Hospital's Front Business Office or at the Clay County Emergency Food Pantry.
Forms must be completed and received no later than: Monday, July 3rd
